The page name moves out of each page's header and into the window toolbar, right of the sidebar toggle, where it stays on every page (as in Finder and System Settings).

  • Toolbar: the page name is always shown and is the page's h1. The scroll-driven fade (PageTitleContext, headerSeen) is removed.
  • PageHeader: no title prop. It starts with the summary line; the summary and actions are vertically centred on one row. With no summary, actions or tabs it keeps only the top margin.
  • Removed the per-page title strings that only fed the header.
  • src/ui/README.md and the type-scale notes in index.css updated.

Most of the App.tsx diff is the shell losing one level of indentation (git diff -w shows the real change).

Checked in the preview harness: every page in dark mode, the empty routing page, the loading skeleton and the English UI. tsc (app + shots) and vitest pass.

Product screenshots (README / website) are now out of date and will be retaken with pnpm shots before the next release.

The page name is now written once, always, in the 38px window toolbar right of
the sidebar toggle (as an h1). PageHeader loses its title and starts with the
summary line; the title-visibility context that faded the toolbar name in on
scroll is gone, along with the per-page title strings it needed.
