Bug:
Currently, vLLM crashes with a cuMemcpyBatchAsync segfault when using native KV offloading (--kv-offloading-backend native) alongside MTP speculative decoding on long contexts.

Root cause:
I found that the OffloadingConnectorScheduler was accidentally including EAGLE/MTP draft-attention groups in both the load and store scheduling paths. Since these draft groups don't have stable block hashes and use tiny gpu_block_size values, it caused a massive miscalculation (num_gpu_blocks = cdiv(420K, small_size) = 105022). This resulted in an out-of-bounds GPU block index that triggered a silent, hard-to-debug CUDA segfault in swap_blocks_batch.

Fix:

  • Added an is_eagle_group flag to GroupOffloadConfig so we can easily spot these groups.
  • Updated the scheduler to skip eagle groups entirely in all load/store scheduling loops.
  • As a defense-in-depth measure, I also added a pre-DMA bounds check in gpu_worker.py. This ensures that if any out-of-bounds block ID ever slips through in the future, it safely throws a clear Python RuntimeError instead of a silent CUDA segfault.

Accountability:
I used some AI assistance to help me track down the root cause and write the patches, but I have manually reviewed every changed line to ensure it's correct. I am relying on the GitHub CI to run the full Linux suite since I am developing on Windows.

Testing:
Verified the fix locally via a custom standalone regression test suite (8 passing tests ):

  • Confirmed is_eagle_group correctly populates and defaults to False (preserving backward compatibility).
  • Verified the load path (update_state_after_alloc) and store path (_build_store_jobs) appropriately skip MTP groups and correctly return group_sizes=0 and block_indices=0.
  • Verified the defense-in-depth bounds check safely catches out-of-bounds IDs before triggering the CUDA call.

My Claude took back his previous suggestion:

  In the coordinator, "flagging a group as eagle" means: drop the last block during prefix cache lookup. That's a minor
  optimization trade-off — flagging all groups conservatively just means you lose one block of cache hit per group. The
  cost of a false positive is tiny.

  In the offloading scheduler, "flagging a group as ineligible" means: skip all offload operations for that group
  entirely. A false positive means an entire group's KV data never gets offloaded or loaded back — the feature is
  silently broken for that group.

  So the same fallback pattern has vastly different blast radii in the two contexts. The coordinator can afford to be
  conservative (flag everything) because the penalty is small. The offloading scheduler cannot.

  My original suggestion was imprecise — I said "use the same heuristic as KVCacheCoordinator" without thinking through
  how much more destructive it is here. The author followed my advice faithfully, which is fair. But the right answer
  is:

  Don't use a fallback at all. Only skip groups explicitly annotated with is_eagle_group = True.

Which model is triggering this bug?

@orozery it is caused by the interaction between native KV offloading and MTP speculative decoding. On models like deepseek-ai/DeepSeek-V3 that use EAGLE draft heads, the draft attention blocks are very small. When these blocks are offloaded, the block count calculation becomes excessively large, which later causes an invalid block ID lookup and crashes in cuMemcpyBatchAsync.

Yes! I wrote a standalone test reproducing the state of the scheduler when an MTP draft group reaches update_state_after_alloc and _build_store_jobs. I verified that with offload_eligible = False, the scheduler completely skips the group (correctly setting group_sizes=0 and block_indices=0) and the illegal out-of-bounds DMA transfer is no longer dispatched.

@JOSH1024 I dug a bit in the KV cache manager code and I saw that eagle groups do get cached and re-used for hits.
It's just that eagle groups need different handling (that the offloading connector currently lacks).
I asked Claude to summarize it:

1. Detection — store eagle flag on GroupOffloadConfig

SchedulerOffloadConfig.from_spec:
- Read is_eagle_group from kv_cache_config.kv_cache_groups
- Fallback: if use_eagle and no group is explicitly flagged, mark ALL groups as eagle
- Store as a boolean on GroupOffloadConfig

2. Lookup — per-group eagle handling

a. Multi-group convergence loop (_lookup):
- Track eagle_verified: set[int] across iterations
- For each eagle group, only apply the pop if group_idx not in eagle_verified
- Increase max_hit_size_tokens by one offloaded_block_size before querying an eagle group (so the lookup can find the extra block that gets popped)
- After applying, add to eagle_verified
- Clear eagle_verified when a non-eagle group shrinks num_hit_tokens

b. Full attention groups (_maximal_prefix_lookup):
- Reduce returned num_hit_blocks by 1 if eagle

c. Sliding window groups (_sliding_window_lookup):
- Increase required contiguous window by 1 if eagle (or reduce result by 1)

3. Store — exclude the trailing block for eagle groups

In _build_store_jobs, the first loop computes num_blocks per group and collects new_offload_keys to store. For eagle groups, reduce num_blocks by 1 — this excludes the last block from the store candidate set.

The second loop (which builds src_block_ids, group_sizes, block_indices for the actual transfer) doesn't need a separate change — it iterates over keys_to_store which is derived from new_offload_keys, so if the last block's key was never added, it won't appear in the transfer.

All preceding blocks in eagle groups are stable and get offloaded normally. Only the trailing block is volatile (rewritten by the drafter each step) and must be excluded.

- Eagle pop applies once per convergence cycle (guarded by eagle_verified)

yes @orozery ,you're absolutely right that skipping the entire Eagle group was too aggressive.

I've pushed an update that follows the approach you described:

  • Restored is_eagle_group with the fallback logic in from_spec.
  • Updated _lookup to track eagle_verified, extending the lookup window by one block for Eagle groups and then subtracting that volatile block from num_hit_blocks. It also clears eagle_verified when a non-Eagle group shrinks the hit boundary so the logic can be re-evaluated correctly.
  • Removed the Eagle-group skip in update_state_after_alloc, so Eagle groups are now loaded normally.
  • Updated _build_store_jobs to reduce num_blocks by one for Eagle groups during job construction, which naturally excludes the trailing volatile block from both keys_to_store and the resulting transfer jobs.

With this change, Eagle groups should be cached and reused as normal, while only the trailing volatile block is excluded from cache storage and transfer.

This was taken from the exact heuristic used in KVCacheCoordinator, which specifically targets only the final group because MTP models typically use the last group for draft tokens, and falls back to 0 if there's only one group. However, as you pointed out, applying it to all groups here is a much safer default.

My bad, I was blindly bringing over the exact fallback logic from KVCacheCoordinator (which intentionally targets just the last MTP layer). Since missing a volatile group in the offloading scheduler causes a crash, conservatively flagging all groups makes much more sense. I've updated it to use the full range in the latest commit!

@orozery I reviewed the failing jobs and don't see a clear connection to this change. The distributed failures appear to originate from a torch.distributed initialization issue (EADDRINUSE) and subsequent DP1 worker termination. The new fields (kv_cache_size_tokens, kv_cache_max_concurrency) are defined in CacheConfig with None defaults and propagated through optional fields in EngineCoreReadyResponse, so I don't see an obvious code path from this diff to the reported failures. A rerun of the Distributed DP Tests job would help determine whether this is CI flakiness. If DP1 continues to fail consistently, the full log before the coordinator shutdown would be needed to identify the actual root cause.

@orozery The AMD test_spec_decode_logprobs[ngram-*] failure is pre-existing and unrelated to this PR. This test has a well-documented history of ROCm non-determinism flakiness (see PRs #34599, #35043, #41335). The test itself has an open TODO acknowledging that wvSplitK kernel non-determinism across LLM instantiations hasn't been fully resolved yet. The logprob diff of 0.2148 (vs abs_tol=0.1) appearing identically for both raw_logprobs and processed_logprobs variants on the same token is characteristic of this hardware-level issue, not a regression. This PR's changes to core.py (check_for_draft_tokens) are logically equivalent to the old use_spec_decode condition for all spec decode cases, and the deferred scheduler output path (where the only behavioral difference is) is only triggered by structured output with grammar masks — not by this test.

The vllm_c rms_norm/fused_add_rms_norm guards claimed support for
weight=None, but torch.ops._C.rms_norm cannot take a None/undefined weight
(fails with 'Not yet supported ScalarType'). Weightless norms (e.g. Gemma4
v_norm, has_weight=False) now correctly fall back to the native impl.

* test(steering): retarget key-coercion test at coerce_steering_spec

The SetSteeringRequest.vectors field is intentionally dict[str, Any] (to
admit the packed wire form), so the model does not coerce inner layer keys;
coerce_steering_spec does. Test the actual coercion seam (which had no
direct coverage) instead of obsolete model-level behavior.

* fix(capture): skip broken consumer entry points instead of crashing

A single third-party capture-consumer plugin that fails to import (e.g.
one referencing a module not present in this build) previously crashed
_load_entry_points and took down all capture admission. Skip it with a
warning so other consumers keep working.
