Rick Bowman liked thisRick Bowman liked thisHomebuyers in America's largest cities are paying thousands of dollars a year more for their mortgages than they need to — and the bigger the market, the bigger the penalty. In my latest story for Bankrate, I looked at how much homebuyers are shelling out in excess mortgage costs in the 10 largest U.S. metros. Los Angeles is the worst of the 10: 83% of borrowers overpay, by an average of $8,139 a year, or nearly $150,000 over a 30-year loan. Miami comes in second at $6,358, then New York at $6,012, Washington at $5,816 and Dallas at $5,320. Nationally, borrowers overpay for their mortgage by $3,343 on average. What our exclusive research at Bankrate really shows is how much mortgage borrowers in expensive markets stand to lose. Half of a percentage point may not sound like much, but on a pricey home purchase, that's thousands of dollars a year and tens of thousands over time in unnecessary interest. To get this data, we compared 3.2 million 2025 HMDA loan originations against binding, competitive offers available at the time on Bankrate's marketplace, controlling for over a dozen factors that affect loan pricing. So why does this keep happening? Many buyers don't have the tools to effectively shop for a mortgage (a complex financial product), housing experts say. Mortgage lending is price transparent on paper, but buyers often have no way to tell whether the offer in front of them is a good one. And it's easy to get overwhelmed in the process: 49% of buyers seriously consider only one lender, according to the National Survey of Mortgage Originations. Real estate agents also tend to recommend lenders they trust to close on time, especially in competitive markets. We’re not taught about mortgages in school; we enter adulthood armed with knowledge of how to find the area of a circle and that the mitochondria is the powerhouse of the cell. When it’s time to buy a house, we’re often left to fend for ourselves. If you’re lucky, you have a competent loan officer or broker in your corner. If you’re not, you could be steered toward a loan that’s more expensive than it should be. And the process doesn’t end once you’ve signed on the dotted line; most mortgages are 30-year contracts, and a lot can change in that time. Interest rates will rise and fall. Knowing how — and when — to refinance your mortgage can help you take advantage of those dips. Your life’s circumstances can change, too, which might require you to make a change to your mortgage. AI local-recommendation audits need to record whether live retrieval was available. Otherwise, recalled names and verified providers get mixed into the same result. A new study compared recommendations with Medicare and SEC registries across the 100 largest U.S. metropolitan areas. Without web search, only 4% of open-weight doctor recommendations and 11% of proprietary-model recommendations matched a clinician in the queried city. With search enabled, 64% to 71% matched real providers in the relevant domains. The study also reports that advisory firms recommended without search had SEC misconduct disclosures at 3.6 times the registry base rate after adjusting for firm size. For restaurants, recommendation favored businesses with three to five times as many reviews, while the rating premium was no more than 0.1 stars. When you put many data points together, some interesting patterns emerge. Medicare Part B enrollment is lower than before. But the total Part B payments are up. The physician fee schedule has been declining. But the services per beneficiary are increasing. The physician services are not the "main" driver. **Continuous improvement in clinical applications is rarely about adding more features.** It is about removing friction from the work clinicians already do. During one EMR implementation, we found that nurses were spending several minutes documenting a routine medication administration because the workflow required too many screens. On paper, the change looked small. We simplified the sequence, tested it with the ward team, and reduced the process to a few focused steps. The impact was larger than expected: less duplicate entry, fewer missed fields, and more time for patient care. That experience stayed with me. A clinical application should improve through regular observation—not occasional grand redesigns. We need to watch how doctors, nurses, technicians, and patients actually use the system. Then fix one pain point at a time. The same principle applies to AI, GenAI, and interoperability. A FHIR API or an AI assistant is valuable only when it makes care safer, faster, or clearer. Technology should fit the clinical workflow—not force the workflow to fit the technology. After 22+ years across hospitals, EMR programs, digital transformation, and healthcare technology, I believe the best improvements often come from listening carefully to the people closest to care. Hot take: betting or investing in any niche EHR right now (e.g. AI-native EHR for X clinical specialist or X vertical like Longevity) when AI code development exists and only continues to get better with each new model release has to be dumbest pre-AI SaaS era thing an investor can do. Here's why ⤵️ The EHR market is essentially split into 5 categories: #1. Outpatient EHR-enabled private practice (doctor or clinical owned) #2. Outpatient tech-enabled practice (PE, VC, family office owned) #3. Outpatient tech-enabled practice (MBA or bootstrapped owned) #4. Startup enterprise (PE or VC backed) #5. Inpatient hospital VC backed EHRs primarily make their money selling to #2-4, with the Epic's of the world taking most if not all of 5. For #1 there really isn't any true TAM since it's difficult to acquire this customer even when giving the EHR product away for free. But here's what these EHRs are not telling you and why they're all racing to update their API docs. They hope that #2-4 will continue to use their solutions to build with AI vs exiting and vibe coding their own in-house solutions. So when you hear them say their EHR product is safe from AI, that's not true. Right now the Epics of the world are safe but it's customers in categories 1-4 who will 100% vibe code their own solutions. #VibeWithCline #LetDoctorsVibe

If you are an EMR without an open/"public" API strategy... because you've arguably been following the Epic playbook for years... well, that playbook just changed. (I need to clarify that said strategy should not require doubling the price either in order for clients to "pay to play") Health tech of the future is (finally) going to look very different than it does today. I have a vision for this. For the first time, I can actually say that I think Epic might be getting on board with it ;)
